چکیده

We appreciate Gu’s [1] interest in our study. We apologize and agree with his comment about attributing units to standardized mean difference (SMD). Nevertheless, similar to the SMD, results in mean difference (control – nebulized) were unaffected by nebulized antibiotics (2.67 days, 95 % confidence interval (CI) –2.89, 8.23 for ICU length of stay (LOS); and 0.70 days, 95 % CI −3.40, 4.80 for mechanical ventilation). However, we strongly disagree with other points raised by the letter. First, the study protocol was defined a priori [2]. We disagree that combining observational studies with intervention studies is reserved only for safety evaluation. This topic has been discussed in the literature and combining both types of studies was adequate for our aim [3]. Furthermore, we presented the main results separating interventional studies from observational studies, thereby allowing the reader to interpret both analyses independently.

Nebulized antibiotics for ventilator-associated pneumonia: a systematic review and meta-analysis

INTRODUCTION Nebulized antibiotics are a promising new treatment option for ventilator-associated pneumonia. However, more evidence of the benefit of this therapy is required. METHODS The Medline, Scopus, EMBASE, Biological Abstracts, CAB Abstracts, Food Science and Technology Abstracts, CENTRAL, Scielo and Lilacs databases were searched to identify randomized controlled trials or matched obs...

Treatment Protocol of Ventilator-Associated Pneumonia based on Microbial Susceptibility in Pediatric Intensive Care Unit, Isfahan, Iran

Background Choosing a unique empiric treatment for ventilator associated pneumonia (VAP) can be challenging. We aimed to determine the antimicrobial susceptibility pattern of Intensive Care Unit (ICU) of the only referral pediatric hospital in Isfahan in order to design the optimal empiric treatment protocol. Materials and Methods: </strong...

The Development of a Critical Appraisal Tool for Use in Systematic Reviews: Addressing Questions of Prevalence

Background Recently there has been a significant increase in the number of systematic reviews addressing questions of prevalence. Key features of a systematic review include the creation of an a priori protocol, clear inclusion criteria, a structured and systematic search process, critical appraisal of studies, and a formal process of data extraction followed by methods to synthesize, or combin...

Nebulized colistin for treatment of ventilator-associated pneumonia caused by multidrug-resistant Gram-negative bacteria: we still need to straighten out the dose!

We applaud the extensive systematic review and metaanalysis by Zampieri and colleagues on inhaled antibiotics for ventilator-associated pneumonia (VAP) [1] but would like to add some comments regarding inhaled colistin for treatment of VAP caused by multidrugresistant Gram-negative bacteria (MDR-GNB). In patients with cystic fibrosis, the equivalent of 160 mg colistimethate sodium (CMS) produce...

بروز پنومونی مرتبط با تهویه مکانیکی و عوامل خطر مرتبط با آن در بخش‌های مراقبت ویژه

Background: Ventilator-Associated Pneumonia is the most important cause of mortality of nosocomial infections. Still incidence of ventilator-associated pneumonia and its related risk factors in routine nursing care in the intensive care unit has not been studied. The aim of this study was to determine the incidence of Ventilator-Associated in intensive care units and its relationship with risk ...
